What Are Form 1 and Form 1A

Both forms are prescribed under the Central Motor Vehicles Rules 1989, Rule 5. They state your physical fitness to drive. The difference: who fills them.

  • Form 1: A self-declaration. You confirm in writing that you do not suffer from any condition that would make driving dangerous. Signed by you alone.
  • Form 1A: A medical fitness certificate. Filled and signed by a registered MBBS doctor after physically examining you.

  2. Click Downloadable Forms: From the lower menu or footer.
  3. Find Form 1 and Form 1A: Listed under Driver Licence Forms.

  5. Print on A4 paper: Standard A4, both sides if needed.

How to Fill Form 1

Form 1 is a one-page self-declaration. Fill these fields:

  1. Applicant name: exactly as on Aadhaar
  2. Father / Husband name: as on Aadhaar or PAN
  3. Address: current residential address
  4. Vehicle class: tick the classes you are applying for (MCWG, LMV, etc.)
  5. Health declarations: tick 'No' to questions about epilepsy, vertigo, fainting attacks, heart disease, vision problems, hearing problems
  6. If 'Yes' on any: Form 1 is not enough; you must use Form 1A instead

  4. Take applicant section filled: fill the upper half with your details before the visit
  5. Doctor examines: eyesight, colour vision, hearing, BP, weight, history (30 to 60 mins)
