When Affidavit Is Needed
An affidavit is a sworn statement signed before a notary or magistrate. It is required in these scenarios:
- Name correction without gazette: affidavit explains the discrepancy and confirms identity
- DOB correction: sworn statement that birth certificate / 10th marksheet shows the correct date
- Surname change after marriage: sometimes accepted in place of gazette in select states
- Multiple changes at once: many states want a single consolidated affidavit listing all corrections
- Discrepancy with Aadhaar: if Aadhaar already shows wrong data, fix Aadhaar first; affidavit supports the bridging
Standard notary affidavit costs approximately Rs 100 to Rs 500 in metros. The format is available at the notary's office or downloadable from state Government websites.
Supporting Proof Documents
| Correction | Primary Proof | Backup Proof |
|---|---|---|
| Name spelling | Aadhaar (corrected) | PAN, passport, 10th marksheet |
| Date of birth | Birth certificate | 10th marksheet, passport, school transfer certificate |
| Father's name | Aadhaar (correct), 10th marksheet | Passport, voter ID with father's name |
| Surname after marriage | Marriage certificate + gazette | Updated Aadhaar with new surname |
| Full name change (deed-poll) | Gazette notification | Affidavit, newspaper public notice |
| Blood group | Pathology lab report | Hospital discharge summary |
Mistakes That Hold Up Corrections
- Aadhaar mismatch: if Aadhaar still shows the wrong name or DOB, update Aadhaar first; otherwise correction is rejected
- Missing gazette: formal name change cases without gazette notification are routinely returned
- Birth certificate from non-recognised body: only birth certificates from registered municipal body or hospital are accepted
- Old affidavit: affidavits older than 6 months are sometimes flagged; get a fresh one if applying late
- Single correction split into multiple applications: consolidate all corrections (name + DOB + photo) into one application to avoid duplicate fees
Correct Records and Insurance
A name or DOB mismatch between your DL, Aadhaar, vehicle RC, and motor insurance policy can hold up claim processing by days or weeks. The surveyor checks all four documents and any discrepancy raises a fraud-check flag. Once you correct your DL, update Aadhaar, RC, and the insurance policy in the same week. Set a single calendar reminder for the cycle. Compare car insurance plans and update your records when you renew.
